Full Job Description
Join our client's Home Health team as a Physical Therapist and make a meaningful impact on patients' lives. In this role, you'll deliver exceptional Physical Therapy services directly to patients in the comfort of their homes. Key Responsibilities:
- Conduct Physical Therapy assessments and develop personalized treatment programs
- Document and report patients' responses to treatment and changes in condition
- Educate patients and caregivers on therapy programs and device usage
- Supervise physical therapy assistants and, when needed, certified nursing assistants
- Prepare comprehensive reports and actively participate in staff and agency development activities
Schedule and Shift Details:
- Monday to Friday; 8:00 am/9:00 am to 5:00 pm
- One weekend per month (Saturday & Sunday; day shift with scheduled patients)
- Two holidays (one summer, one winter); no on-call duties
Travel:
- Local travel to residential homes
**This is a pay per visit position
Requirements
- Master's degree or higher in Physical Therapy
- Minimum 1 year of experience in a healthcare setting
- Valid Physical Therapist license in the state of Illinois (IL)
- Valid regular driver's license
