Physical Therapist Home Health

Seasoned Recruitment is currently seeking a compassionate and motivated Physical Therapist to join our dedicated team of healthcare professionals in Chicago, IL. As a Physical Therapist in our Home Health division, you will have the opportunity to make a positive impact on the lives of patients by providing quality care in the comfort of their own homes.

Responsibilities:

• Perform comprehensive evaluations and assessments of patients' physical conditions and functional abilities
• Develop individualized treatment plans and goals based on evaluation findings
• Implement and modify treatment plans as necessary, utilizing evidence-based practice
• Provide hands-on therapy and instruction to patients to improve mobility, strength, and overall function
• Educate patients and their families on proper techniques for exercises and activities
• Monitor and document patient progress and communicate effectively with the healthcare team
• Adhere to regulatory and ethical standards in all aspects of patient care
• Full-time, 40 hours/week local travel to patient homes

Schedule:

• Monday-Friday 8a-5p, one weekend (S & S every 6 weeks)
• No on-call

Requirements

Requirements:

• Graduate of an accredited Physical Therapy program
• Current state licensure as a Physical Therapist
• At least 1 year of experience in a home health or inpatient setting is required
• HCHB & OASIS experience a plus!
• Excellent clinical and problem-solving skills
• Strong verbal and written communication skills
• Ability to effectively collaborate with interdisciplinary team members and build rapport with patients
• Demonstrated commitment to providing high-quality, patient-centered care
