In this role, you will work collaboratively with a team of therapists and healthcare professionals to provide hands-on physical therapy services to patients in their homes. You will assist in implementing and modifying treatment plans, monitor and record patient progress, and educate patients and their families on proper techniques for exercises and activities.

Responsibilities:

• Assist in implementing individualized treatment plans for patients in accordance with the physical therapist's direction
• Guide and assist patients with therapeutic exercises and activities to improve mobility and function
• Monitor and record patient progress during therapy sessions
• Educate patients and their families on proper techniques for exercises and activities
• Assist with modalities such as heat, cold, and electrical stimulation under the direction of the physical therapist
• Collaborate with the care team to optimize patient outcomes
• Maintain accurate documentation of patient treatment plans and progress

Requirements

Requirements:

• Graduation from an accredited Physical Therapist Assistant program
• Licensure or certification as a Physical Therapy Assistant in the relevant state
• Prior experience in a healthcare setting, preferably in home health
• Valid driver's license and reliable transportation
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
