Job Summary:
As a Registered Nurse (RN) at Loyal Care, LLC, you will play a critical role in providing comprehensive nursing care to clients in their homes. You will assess client needs, develop and implement care plans, and ensure the delivery of high-quality care. This position requires a compassionate, detail-oriented individual with strong clinical skills and the ability to work independently.
Key Responsibilities:
- Client Assessment: Conduct initial and ongoing assessments of clients' physical, emotional, and functional needs.
- Care Plan Development: Develop and update individualized care plans based on client assessments and in collaboration with the care team.
- Direct Patient Care: Provide skilled nursing care, including wound care, medication administration, and monitoring vital signs.
- Supervision of Caregivers: Supervise and guide CNAs and other caregivers in delivering client care according to the care plan.
- Client and Family Education: Educate clients and their families on disease management, medication administration, and other aspects of care.
- Documentation: Maintain accurate and timely documentation of client care, changes in condition, and communication with other healthcare professionals.
- Coordination of Services: Collaborate with other healthcare providers, such as physicians, therapists, and social workers, to ensure comprehensive care.
- Emergency Response: Act as the first responder in emergency situations, providing immediate care and coordinating with emergency services if necessary.
- Compliance: Ensure that all care provided complies with state and federal regulations, as well as Loyal Care, LLC policies and procedures.
- Continuous Improvement: Participate in ongoing training and professional development to stay current with best practices in home healthcare.
Requirements
Qualifications:
- Licensure: Valid Registered Nurse (RN) license in Georgia.
- Experience: nursing experience, preferably in home care, geriatrics, or chronic care management.
- CPR/First Aid: Current CPR and First Aid certification.
- Background Check: Must pass a criminal background check and drug screening.
- Driver's License: Valid driver’s license and reliable transportation.
- Skills: Strong clinical, communication, and organizational skills; ability to work independently and make sound decisions.
- Physical Requirements: Ability to perform physical tasks such as lifting, moving, and transferring clients as needed; ability to stand, walk, and bend for extended periods.
