Entegrity's Homecare Solutions is currently seeking experienced and compassionate LPN's to join our team. As an LPN, you will provide high-quality nursing care to our clients in their homes. This role requires strong clinical skills, excellent communication, and the ability to work independently in a home setting.

Responsibilities:

• Administer medications and treatments as prescribed by the healthcare team
• Monitor and assess client's condition and report any changes to the healthcare team
• Assist clients with activities of daily living, including personal care, mobility support, and meal preparation
• Educate clients and their families on healthcare management and disease prevention
• Document client care and maintain accurate records
• Collaborate with other healthcare professionals to develop and implement care plans
• Provide emotional support and compassionate care to clients and their families

Requirements

Requirements:

• Valid LPN license and registration in the state
• Prior experience as an LPN in a home care or similar setting
• Strong clinical skills and knowledge of nursing practices and procedures
• Excellent communication and interpersonal skills
• Ability to work independently and make sound judgments
• Compassionate and caring nature
• Reliable and responsible
• Flexibility to work various shifts, including weekends and holidays
