The Executive Director will oversee the operational management for the Marti Health Specialized Medical Home with shared responsibility for planning, policy formation, and strategic implementation of programs or services. They will be the organizational source of knowledge of the administrative, logistical, and operational tasks related to the program. The selected candidate will lead policy and regulatory compliance and manage other professional staff, collaborating with internal and external constituents to achieve overall strategic goals. The Executive Director has the responsibility of complex assignments; ability to identify options and develop solutions for situations that will improve the Marti Health operations; senior-level decision-making authority and the judgment, resourcefulness, and ability to identify issues and address them accordingly. This role will require resources for others, participating in mentoring, coaching, and training. The Executive Director reports directly to the Chief Health Officer/Chief Medical Officer for Marti Health.

Requirements

Executive Management Responsibilities:

  • Manage an interdisciplinary specialized medical home care management team providing medical, behavioral and health related social needs care and coordination for individuals with SCD to improve quality and outcomes of care, increase access, reduce workforce strain, lower costs and advance health equity for Marti Health care recipients. This includes oversight on appropriate personnel, programs, internal meetings with key partners within the Marti Health care ecosystem and any matters related to individuals and personnel within the sickle cell specialized medical home.
  • Execute the SCD specialized medical home policies and procedures and oversee care coordination and case management activities working the team.
  • Work with the Marti Health team and stakeholders to ensure adherence to evidence-based best practice guidelines to improve the quality of SCD care.
  • Participate in efforts to ensure highest quality data reporting, tracking and performance evaluation to measure and monitor process and health outcomes.
  • Maintain, review and develop quality improvement strategies to indicate needs of the program.
  • Ensure onboarding, credentialing and other personnel matters are completed in a timely manner.

Customer Support Responsibilities:

  • Oversee the day-to-day responsibilities of the staff to ensure that the needs of the specialized medical home customers are addressed in a clinically competent, culturally sensitive manner.
  • Work with Marti Health executive leadership team to ensure communication of new initiatives, roles and responsibilities of staff, as well as concerns, and questions.
  • Stay abreast of policy notifications and changes that may impact the medical, behavioral and health related social needs care team approach to the customer.
  • Maintain contact with care customers and their families to ensure their needs are addressed by medical and behavioral health teams.
  • Work with the Nurse case manager to address any patient reported concerns as needed.

Communication and Program Support Responsibilities:

  • Utilize reports including hospital activity and patient engagement to contact and/or visit patients directly to discuss program access, prevention services and utilization of services.
  • Monitor documents of patients contact in approved documentation system.
  • Demonstrate and practices HIPAA requirements when sharing patient specific information.
  • Effectively communicate with the Chief Medical Officer and/or team on any necessary information on patients to ensure that the customers outpatient and inpatient care teams are informed on appropriate matters related to patient care. These include partnerships with the ED, outpatient clinic and outside programs (e.g. food insecurity or housing instability programs), community-based organizations, including the Sickle Cell Foundation of Georgia and others.
  • Performs other duties as assigned to ensure the successful operation of the Marti Health specialized SCD program and update all training as necessary.

Minimum Requirements:

  • Bachelor’s degree in social work, psychology, or other health related fields with a minimum of 5-7 years program management experience required.
  • Experience in working in a fast-past acute care setting required.
  • Experience in developing tracking mechanisms, documentations, and integrations of technology.
  • Ability to work with upper management administrators and clinical providers.
  • Demonstrated experience working in and fostering a culturally sensitive and compassionate environment and commitment to do so for patients and their families.
  • Self-starter with strong interpersonal skills
  • Ability to work in a fast-paced environment and manage multiple priorities; strong analytic
  • skills and mindset.
  • Demonstrated effectiveness in adapting communication style based on audience needs.
  • Frequent and effective collaboration cross functionally.
  • Exceptional written and verbal communication skills.

Preferred Knowledge, Skills and/or Abilities:

  • Experience working in healthcare or public health settings to lead and implement programs.
  • Previous experience or collaboration with patient navigators, community health workers and social workers in a community setting or acute care setting.
