Physical Therapist License

At THE LAKES HOME CARE, we are seeking a highly skilled and compassionate Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in helping our patients improve their physical well-being and achieve their rehabilitation goals. Responsibilities

• Assess and evaluate patients' physical conditions and develop individualized treatment plans
• Implement therapeutic exercises and activities to improve patients' strength, mobility, and flexibility
• Administer modalities such as electrical stimulation, ultrasound, and heat/cold therapy as part of the treatment plan
• Educate patients and their families on the benefits of physical therapy and provide instructions for at-home exercises and self-care
• Monitor patients' progress and adjust treatment plans as necessary
• Collaborate with other healthcare professionals, including physicians and occupational therapists, to ensure comprehensive and effective patient care
• Maintain accurate and thorough documentation of patients' evaluations, progress, and treatment outcomes

Requirements

• Valid Physical Therapist license
• Bachelor's or Master's degree in Physical Therapy
• Proven experience in providing physical therapy services, preferably in a home care setting
• Strong knowledge of anatomy, physiology, and rehabilitation techniques
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassion and empathy for patients with diverse backgrounds and conditions
• Current CPR certification
• Proficiency in relevant computer applications and electronic medical record systems is preferred
