Full Job Description

We are seeking a friendly, empathetic and detail-oriented Long Term Home Care Coordinator to join our team at Parx Home Care. As a leading home care agency servicing Miami-Dade counties in Florida, our mission is to provide peace of mind to families knowing their loved ones are receiving the best care possible. As our long-term care coordinator, you will be instrumental in improving the quality of life for those requiring long-term care while allowing them to preserve their independence and dignity in their own homes. Your prospective commitment to evaluate, plan, and coordinate care will inevitably result in a favorable health outcome for every client.


Responsibilities

  • Establish, nurture and maintain relationships with Long-Term Care Case managers of leading insurance and Medicaid plans to establish the care plan, coordination of home care services, and obtaining of authorizations for services
  • Proactively reach out to Medicaid Long-term care case managers to let them know our care giver availability by service area
  • Build relationships/partnerships with clients and their families and other healthcare professionals
  • Collaborate with clinical team to adjust care plans as needed
  • Provide updates on clients as necessary to case managers to cultivate collaborative partnerships with providers
  • Ensure services offered complies with legal requirements and meets organizational standards and meets the needs of our long-term care plan partners
  • Work with clients and families to provide on-going assistance and information pertaining to services and care plan
  • Address any caregiver relations issues that may arise and escalate to appropriate department
  • Work closely with the scheduling department to ensure that staffing needs are met


Requirements

  • At least 2-3 years of experience in healthcare industry, preferably in the home care
  • Bilingual (Spanish)
  • Exceptional communication and interpersonal skills
  • Strong organizational and time-management abilities
  • Knowledge of employment laws and regulations
  • Ability to work independently and as part of a team
