Full Job Description
We are seeking a friendly, empathetic and detail-oriented Intake and Scheduling Home Care Coordinator to join our team at Parx Home Care. The Intake Supervisor will be the key process owner for our incoming home health referrals, overseeing an intake team covering multiple locations across Florida. This leader will ensure standardization, best practices, and compliance with State guidelines relative to the home health benefit. They will also be responsible for the implementation and supervision of the policies, procedures and workflows used by the Intake Coordinator staff for the direct purpose of obtaining and maintaining eligibility for patients as well as managing all aspects of the patient intake process including overarching clinical standards and practices. Intake builds the foundation for successful billing. This individual is a key stakeholder in the organization and must have the ability to maintain and grow positive relationships with patients, their families, Referral Sources, and internal staff.
Responsibilities
- Directs all daily patient referral, intake, and authorization operations.
- Intake gathers all the pertinent information for the patient up front which makes it possible to successfully bill claims and be reimbursed for patient care.
- Oversight of the insurance authorization process regarding case management and patient pathways. To include timely and complete processing of eligibility and benefit verification, initial and ongoing authorization requests following set guidelines and accurate entry of related information in designated software system.
- Processing of patient intake and coordination of care, end-to-end
- Coordinates with the Clinical and Marketing teams to ensure required documentation is obtained from physicians.
- Supports intake process and team in leading by example and participating in daily work routines and key processes.
- Monitor and evaluate the performance of these processes, making data-driven adjustments as needed.
- Obtains initial and on-going authorization for service requests from non-Medicare pay sources.
- Ensure authorization for services is in place for the ordered treatment periods.
- Reviews the monthly eligibility report and ensures that all insurance corrections are made to patient demographic information.
- Reviewing and managing assigned daily and weekly reports.
- Key liaison to business development team establishing strong processes of communication.
- Actively drives communication processes with patient, family, and referral sources / providers.
- Ensuring patients/ referral sources / agency staff in the referral process are kept informed an updated on the status of authorization.
- Build long-lasting relationships with Branch Leaders to drive strong connectivity and an environment of support and growth.
- Complies and is a role model for all areas of the Agency Compliance Program and HIPAA regulations.
- Maintain effective fiscal management productivity of team by monitoring metrics and publishing of KPIs Leadership, supervision, and support of the Intake Coordinator team.
- Develop and implement strategic plans for the regional intake department, aligning with the organization's goals and objectives.
- Identify opportunities for growth, efficiency, and service improvement within the intake and authorization process.
- Monitor multiple emails and multiple e-referral systems to identify new referrals and manage alerts and notifications. Perform timely follow-up and manage all related communication.
- Design, implement, and continuously improve the intake and admissions processes to ensure efficiency and effectiveness.
- Provide guidance, support, and professional development opportunities to the team.
Requirements
- Strong leadership and team management abilities.
- Strong Knowledge of Skilled Home Health regulations, admission criteria and compliance standards
- Excellent communication and interpersonal skills with demonstrated decision-making ability.
- The employee must be able to think independently and possess excellent interpersonal, organizational and customer service skills.
- Demonstrate ability to solve problems independently and at an optimal level.
- Proven organizational skills with high attention to detail and the ability to prioritize.
- Management and strong abilities with multiple computer information systems, including electronic medical record systems, electronic patient referral systems, and Microsoft Office
