Home Health Occupational Therapist

Thomas Talent Network, LLC is currently seeking a compassionate and skilled Home Health Occupational Therapist to join our team. As an Occupational Therapist, you will provide care and treatment to patients in their homes to help them regain or maintain their ability to perform everyday activities. You will work closely with patients, families, and a multidisciplinary team to develop personalized treatment plans and provide ongoing support and education.

Responsibilities:

• Conduct initial evaluations of patients to assess their needs and goals
• Develop individualized treatment plans based on patient assessments and goals
• Provide skilled therapeutic interventions to improve patient outcomes
• Educate and train patients and their families on adaptive techniques and equipment
• Monitor patient progress and adjust treatment plans as needed
• Document patient evaluations, treatment plans, progress notes, and other relevant information
• Collaborate with other healthcare professionals to coordinate patient care

Requirements

Requirements:

• Valid Occupational Therapist license
• Minimum of 1 year of experience as an Occupational Therapist, preferably in a home health setting
• Strong clinical and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver's license and reliable transportation
• Flexibility to travel to patient homes within the assigned territory
