Home Health Care Liaison

Parx Home Care in Miami, FL is looking for a talented Home Care Liasion to join our amazing team!

The Home Care Liasion will be responsible for overseeing the growth of our agency's patient base. We are searching for a candidate who is knowledgeable about the home care referral process and has the ability to establish strong relationships with various local organizations in order to acquire patient referrals.

What you will be doing:

• Effectively communicate and promote the services provided by Parx Home Care to the community and industry.
• Follow up on leads and referrals from family members, caregivers, and other sources.
• Keep track of referral activities and outcomes and maintain a reference list of sources and patients for future referrals.
• Utilize leads provided by our strategic vendor partners to arrange initial meetings and onsite assessments for potential patients with our intake team.
• Coordinate, communicate, and manage attendance for upcoming home care education and healthcare events aimed at potential patients.

What we're looking for in you:

• At least 1 year of experience in a sales or marketing role in a professional setting, preferably in Home Care and/or Healthcare.
• Outstanding communication skills and a strong business sense.
• Ability to work independently while demonstrating strong leadership skills.
• Proven track record of success in establishing valuable community relationships and delivering presentations to diverse demographics and group sizes.
• Familiarity with Home Care and Medicaid eligibility.
• Proficient in using Microsoft Office Suite
