Full Job Description
We are currently seeking a Care Coordinator - Women's Services to provide quality, sufficient and appropriate care management and advocacy services to clients served at the Sanctuary. The Care Coordinator does this by increasing access to care through outreach and enrollment, improving health through education and support, and appropriate use of health care and behavioral health services. All services are performed on-site and in the community.
Please note this is a full-time Monday-Friday day position with occasional evenings and weekends on-call.
Requires local travel within the community.
Responsibilities
- Evaluate women for eligibility for services within 72 hours of admission.
- Providing culturally appropriate health education.
- Proving cultural mediation between communities and health and human services systems.
- Educate and counsel patients about plans of care, health promotion and disease prevention.
- Conduct periodic updates on health status of clients according to program guidelines.
- Educate client and colleagues on features of health systems that promote the integration and utilization of disease prevention-health promotion services into clinical practice.
- Meets with clients as needed to address physical health, mental health and behavioral health in any/all settings conducive to the client.
- Provides individualized client/family education customized to the client and support system.
- Refer clients to appropriate clinical and/or medical staff as needed within the agency and in the community in an accurate and timely manner.
- Conducts individualized care management needs assessment, prioritizing the data collection based on the client’s immediate condition or needs within timeframe specified by client facility’s policies, procedures.
- Conducts ongoing assessments as determined by the client’s condition and/or the client facility’s policies, procedures or protocols and reprioritizes care accordingly.
- Provides resources and coordinates referrals as necessary and required.
- Provides transportation to appointments.
Requirements
Education and Certification:
- Bachelor’s Degree in Human Services, Social Work or related field
- Certified Alcohol and Drug Counselor (CADC) preferred
Experience:
- 2 years proven work experience as a Care Coordinator or similar role
- Minimum 2 years of work experience in the human services field
Skills:
- Highly organized with a strong ability to manage different priorities
- Ability to work with others and use resources to problem solve
- Apply good judgment to fast-changing situations
- Strong customer service skills
- Effective verbal and written communication skills
- Specialized training or experience in:
- Women’s studies/services
- Trauma informed care
