Physical Therapist - Home Health

Responsibilities:

- Provide high-quality therapy services in accordance with applicable Standards of Practice.

- Conduct patient evaluations and develop personalized treatment plans, setting both short and long-term goals.

- Monitor patient progress, reactions to treatments, and changes in condition; adjust treatment plans accordingly.

- Educate patients, caregivers, and healthcare team members on rehabilitation methods and techniques.

- Maintain regular communication with physicians and other healthcare professionals.

- Develop realistic discharge plans for patients, ensuring smooth transitions to post-treatment care.

- Report any significant patient, family, or physician issues to the Clinical Director.

Requirements

Requirements & Qualifications:

- Current state licensure, CPR certification, driver’s license, and automobile liability insurance.

- Thorough knowledge of professional theory, practice, techniques, principles, and equipment.

- Strong oral and written communication skills; ability to document patient care effectively.

- Ability to establish and maintain positive relationships with patients, healthcare professionals, physicians, and the public.

- New grads are welcome; 1+ year of experience preferred.
