Centrally located in Torrance, California, Sunnyside Nursing and Post-Acute Care provides sub-acute nursing care, rehabilitation services, skilled nursing, and long-term care. We are looking for a Unit Director of Nursing/ Unit Manager (RN or LVN) to lead a 60-bed skilled nursing unit. We require a compassionate and responsible individual capable of providing outstanding quality care and customer service with strong ability to manage staff, residents, and families on the unit.
Job Description
- Review 24-hour Report. Review Staffing. Coordinate with RN Supervisor
- Daily Review of Change of Conditions, New/Re-Admissions, Daily Rounds, and discussing during Clinical Rounds.
- Monitor the Unit’s Risk Management Program and ensure all Incidents are reported timely, investigation reviews are completed daily. Maintain Log for Incident Reports (Med room). Lead Interdisciplinary Team (IDT) incident review. Review Resident on the Falling Star Program. Coordinate with the SNE every month for QAPI reports. May Attend Monthly/Quarterly QAPI Meetings as indicated.
- Review Baseline Care Plan for accuracy and completion for all New Admissions (within 48 hours) and coordinate care with other IDT members for completion of sections and signatures.
- Monitor residents with wounds. Coordinate with Treatment Nurses for interventions, preventative measures and other devices for proper application.
- Monitor staffing Assignments/Schedules and coordinate with the Labor Relations Liaison. Oversee Call-Offs/Replacements. Coordinate with SNE and/or CCO for Overtime determination and authorization.Coordinate Time-Off Requests with Labor Relations Liaison.
- Attend Daily, Weekly, Monthly Meeting as scheduled: IDT Meetings as indicated. Daily Clinical rounds, Department Head Leadership Standup, Behavior Meetings, Wound and Weights Meeting, RNA meetings, Safety Meetings, QAPI Meetings, Risk Management Meetings, Clinical/Administration Team Meetings.
- Manage Family and Resident Concerns. Escalate formal/significant complaints to Facility Leadership. Provide Re-Education/Interventions for Chart Audit Deficiencies and other performance-related concerns.
- Coordinate with the SNE/CCO as indicated. Audit Unit Medication Carts PRN for Administration Completion and Controlled/Narcotic Medications Reconciliations, Blood GLucose Quality Monitoring.
- Meet and Greet with new Admissions to ensure customer satisfaction. And address any concern promptly. Attend Physician Rounds for Kaiser and Optum Rounds with SNE.
- Address concerns/issues timely. Review Licensed Nurses Audits for follow-up and coordinate with Unit Clerk and Medical Records Director. Provide education or counseling if indicated.
- Review all Discharged Medical Records for completion of documentation and other forms. Available to answer calls after hours Other duties as Assigned by SNE and or CCO
- Assist Senior Nurse Executive and Chief Clinical Officer as needed.
Requirements
Must have a current and active California LVN or RN License
A minimum of two year experience in nursing supervision in a skilled nursing facility/long term care
Great working knowledge of PCC
Must be authorized to work in the United States
Shift:
Monday - Friday (8:00-5:00)
Some Weekends as MOD
