Position Overview:

QuickTeam is seeking a compassionate and skilled Social Worker with a California education to join our team remotely. The ideal candidate will provide comprehensive support to our patients and field staff, ensuring they receive the necessary assistance and resources. This role involves various responsibilities, including filling out and managing different types of applications for our patients.

Key Responsibilities:

  • Conduct virtual assessments and develop individualized care plans for patients, addressing their unique needs and circumstances.
  • Provide remote counseling and support to patients and their families, helping them navigate challenges and access appropriate resources.
  • Advocate for patients’ rights and ensure they receive the services they are entitled to.
  • Collaborate with healthcare providers, community organizations, and other stakeholders to coordinate care and support services.
  • Maintain detailed and accurate records of patient interactions, progress, and outcomes.
  • Assist field staff by accurately completing various applications, such as medical, housing, food stamp, and disability applications, and returning them for mailing.
  • Stay updated on the latest regulations and requirements for applications and other social work practices to ensure compliance.
  • Participate in continuous professional development and training to enhance skills and knowledge in social work.

Requirements

Qualifications:

  • Bachelor’s degree in Social Work or a related field from an accredited institution in California.
  • Previous experience in social work, preferably with a focus on patient advocacy and resource coordination.
  • Strong organizational skills and attention to detail.
  • Excellent written and verbal communication skills.
  • Ability to work independently and manage multiple tasks simultaneously.
  • Familiarity with state and federal application processes and requirements.
  • Proficiency in using standard office software and applications.
  • Experience working with diverse populations and understanding their unique needs.
