Pay Range: $20.08 to $24.48/hour

SCHEDULE

Monday – Friday 9:00am-2:00pm, 25 hours per week. Extra hours with occasional weekends/evenings may be required.

JOB SUMMARY

Primarily responsible to support the Social Services Supervisor and assist in meeting the psychological needs of residents residing in Independent, and Assisted Living levels 1&2. The Social Services assistant is responsible for having a working knowledge of Title 22 and MasonicHome guidelines pertaining to required delivery of service and documentation. Social Services assistant will establish, track, and maintain interdisciplinary service planning meetings and related committee meetings, and attend mandatory in-service meetings. The Social Services assistant interacts with all Masonic Home departments.

ESSENTIAL FUNCTIONS

  • Conduct residents’ safety room inspection on a Quarterly, Biannual and Annual basis.
  • Help with voter registration, DMV forms, and community transportation.
  • Sets up weekly Service Plan meetings for residents with the Social Services supervisor and send out reminders to residents and their power of attorneys.
  • Coordinates support group speakers for Low Vision group and Special Friends, reserves conference rooms for service plan meetings. In addition to creating fliers for such meetings and delivering to residents’ rooms.
  • Works with the team in brainstorming on activities for the Social Circle, leads the activity with residents.
  • Transfer deceased resident files from Social Services to the deceased file cabinet.
  • Is responsible to box up and label deceased resident files and arrange to have the boxes of files move to the room the file room.
  • Provide 2 week follow up post orientation on new residents.
  • Scan documents into Matrix Care such as MMSE, GDS, and Capacity forms in resident electronic record.
  • Provide the laundry schedule to new residents and update the schedule as residents move to different levels of care.
  • Maintain and update the fire plan, and POLST forms behind resident’s front door for emergencies.
  • Support the social services team with any resident events.
  • Assist in residents room inspection for items that are reported lost/missing and complete the missing item report form.  
  • File pertaining forms in resident clinic charts.
  • Maintain the work area clean and clear of items that may be a safety hazard.
  • Maintain punctual and regular attendance.

Requirements

  • Two years of Administrative Assistant or Social Services support experience preferred.
  • The ability to accept and utilize professional supervision, consultation, and in-service training.
  • The ability to present concise, meaningful written reports in a timely manner.
  • Manage confidential data with professional discretion.
