Full Job Description
We are currently seeking a dedicated Senior Community Healthcare Engagement Specialist to join our team and assume a critical role in establishing and nurturing robust relationships with healthcare providers to ensure the delivery of high-quality care for our PACE participants. The Senior Community Healthcare Engagement Specialist will be responsible for fostering effective communication, facilitating collaboration, addressing challenges, and supporting expansion efforts.
Key Responsibilities:
- Cultivate and sustain strong, dependable connections with primary care physicians, specialists, and healthcare providers responsible for our PACE participants.
- Organize and lead meetings, outreach events, and training sessions to maintain provider engagement and ensure their awareness of our PACE program offerings, procedures, and expectations.
- Collaborate with our clinical, administrative, and operations teams to amplify provider involvement and ensure seamless care coordination.
- Act as a primary resource for providers, aiding in addressing their inquiries, resolving issues, and ensuring transparent communication with our PACE interdisciplinary team.
- Identify opportunities for enhancing collaboration to consistently strive for optimal outcomes for our participants.
- Collaborate with the quality team to monitor provider performance metrics and contribute to improvement initiatives.
- Stay abreast of industry best practices and regulatory changes that may influence provider engagement, and provide support for necessary adaptations.
- Assist in resolving provider-related challenges in a manner that fortifies relationships and upholds the delivery of high-quality care.
- Contribute to strategies for network expansion and the recruitment of new providers as required.
- Demonstrate flexibility by undertaking additional responsibilities and tasks that contribute to the success of the PACE program.
Schedule and Shift Details:
- Monday to Friday, 8:00 am to 5:00 pm, Hybrid.
Travel:
- Local travel required.
