Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education and more.

The Street Health Outreach program is an innovative initiative designed to identify and assist individuals in Central Alameda County who are experiencing homelessness and face chronic health and mental health challenges. The dedicated Street Health Outreach Team is committed to helping clients achieve housing stability and enhance their overall health and well-being.

This program utilizes a multi-disciplinary team approach to deliver a range of services, including street and venue-based outreach and engagement, assessments, health services, triage, case management, and connections to housing agencies and mental health services. Collaboratively, the program works with all participants to create and implement action plans aimed at improving their health and wellness.

Operating under the supervision of the Street Health Program Manager, in coordination with the Chief Medical Officer, the Street Health Outreach RN Care Manager plays a crucial role. Their responsibilities encompass conducting nursing assessments, providing nursing care such as medical assessments, wound care, skin assessments, administering vaccinations (e.g., flu, Hepatitis A), treating respiratory illnesses, and caring for patients with active substance use disorders. Additionally, this position involves supervising the Community Health Worker and the Behavioral Health Case Manager. The RN Care Manager collaborates with medical providers on behalf of patients, with access to medical consultation via phone when needed.

This is a full-time benefited position working 40 hours per week, typically Monday through Friday with periodic Saturday hours.

Compensation: $80,600 - $83,000, depending on experience.

The pay for a selected candidate is determined by a variety of factors to ensure fair and equitable compensation. These factors include the candidate's experience, education, skills, training, licensure, certifications, and the specific scope of the role. Responsibilities:

  • Administer medical care in unhoused populations and TVHC's provided van.
  • Administration of medications and injections to patients as directed by the providers
  • Referrals to other providers and community health resources.
  • Provide health instruction to patients.
  • Conduct health assessments, provide brief interventions, triage patients, and provide support in navigating health and social services systems.
  • Prioritize individuals with high medical and behavioral health needs and/or Substance Use Disorders for intensive visits to improve health and lift them out of homelessness.
  • In the capacity of a patient advocate, assess the learning needs of patients, develop and implement patient education as appropriate, and act as a resource and contact person for patients.
  • Respond to immediate survival needs as prioritized by clients, e.g., basic needs, problem-solving, health education, and information about health and community services.
  • Provide medical oversight of Community Health Worker and Behavioral Health Case Manager.
  • Provide linkage and referrals to community services as needed and work from an interdisciplinary approach.
  • Represent TVHC at county and community meetings and engage with community stakeholders.
  • Participate with other staff in multidisciplinary staff meetings and other staff processes to achieve program development, staff development, program evaluation, program operations review, and other activities as appropriate.
  • Maintain confidentiality regarding privileged administrative and patient information and complete required documentation in a timely manner.
  • Provide outreach and engagement, health services, and triage, and collaborate with Housing Resource Centers.
  • Build relationships over time via consistent visits with potential patients.
  • Screen and identify individuals with medical and behavioral needs.
  • Participate in regular meetings with Alameda County Healthcare for the Homeless.
  • Provide culturally competent, trauma-informed, and harm reduction-based services.
  • Conduct regular outreach and engagement, building trust with individuals encountered at encampments and other service locations according to the responsive schedule.
  • Perform all duties and services in full compliance with TVHC’s Service Excellence Standards.

Requirements

  • Current, valid California RN license.
  • Current, valid cardio-pulmonary resuscitation card (CPR).
  • Valid CA driver’s license and insurance.
  • Preferably, two years of nursing experience serving patients with complex needs and ideally, in unsheltered populations.

Qualifications

  • Preferred bilingual proficiency in English, Spanish, or other languages.
  • Strong ability to work independently, organize tasks, problem-solve, make decisions, collaborate effectively within a team, and manage multiple priorities.
  • Demonstrated experience in implementing quality improvement processes to enhance client care and meet required health risk outcome measures.
  • Exceptional cultural sensitivity and a proven track record of effectively working with ethnically diverse populations.
  • Sensitivity and understanding of the unique needs of unsheltered individuals facing complex challenges.
  • Proficiency in crisis intervention techniques, including de-escalation.
  • Eagerness to engage with members of the unsheltered community through outreach.
  • Outstanding communication and documentation skills, particularly in gathering patients' medical histories and accurately recording medical information.
  • Effective communication with both patients and staff, demonstrating diplomacy and tact.
  • Strong multitasking abilities and the capacity to thrive in high-stress, fast-paced environments.
  • Thorough understanding of the importance of confidentiality and non-disclosure in compliance with HIPAA standards.
  • Willingness to travel to assigned locations and operate the street health medicine van to and from various service sites as needed.
  • Proficiency in crisis intervention techniques, including de-escalation.
