Registered Nurse- Home Health visits

Thomas Talent Network, LLC is seeking a compassionate and experienced Registered Nurse to join our Home Health team. As an RN specializing in Home Health visits, you will have the opportunity to provide personalized and comprehensive care to patients in the comfort of their own homes.

Responsibilities:

• Conduct initial assessments and develop individualized care plans for each patient
• Perform skilled nursing procedures such as wound care, medication administration, and IV therapy
• Monitor and evaluate the progress of patients in their home environment
• Collaborate with the interdisciplinary team to coordinate and manage patient care
• Provide education and support to patients and their families
• Document patient care and observations accurately and in a timely manner
• Ensure compliance with all home health policies, procedures, and regulations
• Communicate effectively with patients, families, and healthcare team members
• Maintain a safe and clean environment for patients during visits

Requirements

• Active RN license
• Prior experience in home health or a related field
• Current CPR certification
• Strong clinical skills and knowledge
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• Compassionate and patient-centered approach to care
• Attention to detail and strong documentation skills
• Must have a valid driver's license and reliable transportation
• Must be able to pass a background check and drug screening
• Active (unrestricted) nursing license in the State of California
• 1 year (professional) nursing experience in Home Health or Acute Care
• Valid Drivers License w/ Clean DMV
• SOC Start of Care
• Evaluation
• Follow Up 
• Discharge / Re-certification
• 5 days of availability Wed-Sun
