Position Summary

The Program Coordinator II for Enhanced Case Management (ECM) will play a crucial role in supporting the Case Manager II by overseeing the coordination and administration of ECM services. This position is responsible for ensuring the effective delivery of case management services, enhancing client access to resources, and facilitating communication between clients, community partners, and internal teams. The Program Coordinator will also help develop and implement program policies and procedures, ensuring high standards of service delivery.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

    • Program Support and Coordination: 
    • Assist in the development and execution of ECM program policies and procedures. 
    • Coordinate with Case Manager II to ensure comprehensive service delivery and address any issues related to case management. 
    • Facilitate communication and collaboration between clients, community partners, and internal teams. 
    • Client Support and Advocacy: 
    • Support the Case Manager II in conducting client assessments and developing individualized care plans. 
    • Help clients access and navigate community resources and services. 
    • Advocate for clients’ rights and needs within the community, ensuring their concerns are addressed. 
    • Administrative Functions: 
    • Maintain accurate and up-to-date documentation of client interactions and service provision. 
    • Monitor program performance and client progress, providing regular reports to management. 
    • Participate in team meetings, trainings, and professional development activities. 
    • Community and Resource Coordination: 
    • Build and maintain relationships with community partners and agencies to enhance service coordination. 

Requirements

Minimum Qualifications (Knowledge, Skills, and Abilities)

  • Bachelor's degree in social work, psychology, or a related field
  • Minimum of two years of experience in case management or a related field
  • Experience in program coordination or supervision
  • Knowledge of enhanced case management models and practices
  • Strong leadership and team management skills
  • Excellent communication and interpersonal skills
  • Proficiency in using case management software and computer applications
  • Cultural competence and sensitivity to the needs of diverse populations
  • Ability to work independently and in a collaborative team environment

Hourly Salary Range: $22.00-$29.00
