Primary Care Physician

Responsibilities:

• Perform comprehensive in-person assessments and collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans for participants.
• Oversee all aspects of participant care in nursing homes and hospitals, including admissions, discharges, and coordination with medical specialists.
• Provide direction and support to Nurse Practitioners and other IDT members to ensure effective care coordination and delivery.
• Deliver primary care services, including assessment, planning, and continuous medical coverage to participants.
• Collaborate with community-based physicians and IDT in discharge planning from acute or long-term care settings.
• Actively participate in IDT meetings, communicate participant changes, and coordinate 24-hour care delivery.
• Maintain regular visits to nursing home participants based on their needs.
• Participate in rotating night and weekend call schedules as required.

Requirements

Qualifications:

• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
• Master’s Degree or Doctorate Degree in Medicine.
• Board Certification in Internal Medicine or Family Practice.
• Board Certification in Geriatrics preferred.
• Active California Physician License without Restrictions.
• State CDS Certification and DEA Registration.
