Full Job Description
Seasoned Recruitment is seeking a qualified and compassionate Primary Care Nurse Practitioner (FNP or AGNP) to join our dynamic healthcare team. As a Nurse Practitioner, you will provide comprehensive, patient-centered care to a diverse population. Your role will involve conducting assessments, diagnosing illnesses, providing treatment plans, and collaborating with a multidisciplinary team to ensure quality care for our patients.

Salary/Wage base range for this role is $145,000 - $175,000 / year + Bonus + Equity.
Key Responsibilities:
- Perform thorough physical exams and patient assessments including initial, semi-annual and unscheduled assessments and continuous medical coverage to participantsDiagnose and manage acute and chronic illnesses.
- Work closely with other healthcare professionals to coordinate care and IDT to oversee and ensure effective care coordination.
- Document all patient interactions accurately in the electronic medical record (EMR).
- Participate in continuous quality improvement initiatives.
- Must collaborate with physicians and IDT to develop care plans for each participant
- Must collaborate with physicians and IDT in discharge planning from acute or long-term settings
- Help maintain care of participants in the nursing home, making regular visits as dictated by participant need
- Collaborate closely with physicians, community-based PACE physicians, and the Interdisciplinary Team (IDT) to supervise and guarantee effective care coordination.
- At the direction of the Medical Director, review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements
- Take part in rotating night and weekend call
Requirements
Qualifications:
- Graduation from an Accredited School of Nursing
- Certificate as a Nurse Practitioner with Prescriptive Authority
- Active Drug Enforcement Administration (DEA)
- Current knowledge of chronic care/geriatric issues and best practices
- At least 2 years of clinical experience in the senior population
- Must currently be enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
- Valid CA. state drivers license
- Exceptional communication and interpersonal skills
