Job Overview:
We are seeking a dynamic and compassionate Patient Success and Care Manager to lead efforts in delivering exceptional patient experiences and improving care outcomes. This role is pivotal in ensuring seamless coordination between patients, families, and care teams while driving initiatives to enhance satisfaction, engagement, and adherence to care plans. The Patient Success and Care Manager will oversee patient onboarding, chronic care management, and ongoing support, ensuring quality standards are met while fostering a culture of empathy and excellence.
The ideal candidate will have a strong background in healthcare operations, patient relations, and team leadership, with a focus on creating systems that enhance the overall patient journey.
Responsibilities:
- Patient Experience Leadership:
- Design and implement patient success programs to improve satisfaction, engagement, and adherence to care plans.
- Oversee patient onboarding processes, ensuring a seamless transition into care services for chronic and elderly patients.
- Act as a key liaison between patients, families, and care teams to address concerns and resolve issues promptly.
- Care Coordination:
- Manage patient monitoring programs to track progress, identify challenges, and proactively intervene to improve outcomes.
- Collaborate with clinical teams to develop and implement care plans tailored to individual patient needs.
- Monitor chronic disease management efforts, leveraging data to adjust strategies and improve health outcomes.
- Team Management:
- Lead and mentor a team of patient care specialists, providing guidance, training, and performance evaluation.
- Foster a culture of empathy and accountability within the team to ensure consistent delivery of high-quality care.
- Program Management and Compliance:
- Oversee the development and execution of patient success initiatives, ensuring alignment with organizational goals.
- Ensure compliance with healthcare regulations, including HIPAA, and uphold the highest standards of patient confidentiality and care.
- Monitor key performance indicators (KPIs) for patient success and care delivery, providing regular reports to leadership.
Requirements
Qualifications:
- Bachelor's degree in Nursing, Social Work, Healthcare Administration, or a related field.
- 8+ years of experience in patient care coordination, healthcare operations, or a related field.
- Proven track record in enhancing patient satisfaction and managing chronic care programs.
- Exceptional communication and interpersonal skills with the ability to build trust and rapport with patients, families, and care teams.
- Strong problem-solving and critical thinking abilities with a focus on patient-centered solutions.
- Proficiency in healthcare technology, including EMR systems, patient monitoring tools, and CRM platforms.
Preferred Qualifications:
- Master’s degree in Healthcare Administration, Nursing, or a related field.
- Certification in healthcare quality, patient advocacy, or care coordination (e.g., IHI, CPXP).
- Experience with patient satisfaction improvement programs in chronic or elderly care.
