As the Office Intake Coordinator, you’ll be the first point of contact for new clients and their families. Your responsibilities will include:

  • Providing information and support to clients and their families about our services.

JOB DESCRIPTION SUMMARY

Responsible for managing all aspects of the patient intake process, establishing and maintaining

positive relationships with customers and referral sources, responding to customer requests and

concerns, and managing the insurance verification and authorization processes. Collaborates and

provides coverage for scheduling as needed.

Requirements

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Assumes all daily patient referral and intake operations including implementation of intake policies.

2. Collaborates with scheduler to assign appropriate clinicians to cases.

3. Provides coverage for scheduling when needed.

4. Utilize Clinical Managers as a resource for intake and scheduling issues.

5. Ensures compliance with all state, federal, and ACHC referral/intake regulatory requirements.

6. Directs the implementation of improved work methods and procedures to ensure patients are

admitted in accordance with policy.

7. Establishes and maintains positive working relationships with current and potential

referral sources.

8. Collaborates with Home Health Coordinator to ensure seamless transition of patients to home by

providing patient education and preparation for home care, plan of care initiation, and coordination

of care with multiple service providers.

9. Ensures maximum third party reimbursement through direct oversight of insurance verification and

authorization processes.

10. Builds and monitors community and customer perceptions of Nursing & Rehab At Home as a high

quality provider of services.

11. Gathers, collates, and reports referral statistics including key customer referral trends.

12. Provides input in strategic planning including identifying opportunities for additional referral

resources or improved services to meet customer needs.

13. Maintains comprehensive working knowledge of Nursing & Rehab At Home contractual

relationships and ensures that patients are admitted according to contract provisions.

14. Maintains comprehensive working knowledge of community resources and assists referral sources

in accessing community resources should services not be provided by Nursing & Rehab At Home.

POSITION QUALIFICATIONS

1. Bachelor’s degree in related field preferred.

2. At least one (1) year experience in health care, preferably in home care operations.

3. Ability to market aggressively and deal tactfully with customers and the community.

4. Demonstrates good communication, negotiation, and public relations skills.

5. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job

responsibilities.

Environmental Conditions: May be exposed to extremes of heat and cold in all weather

conditions. Must drive in various weather conditions on roads in varying degrees of repair.

Working Conditions: May be exposed to infections and contagious diseases. Contact with patients

under wide variety of circumstances. May be exposed or occasionally exposed to patient

elements. Subject to varying and unpredictable situations. Handles emergency or crisis

situations. Travel required. OSHA exposure category: Category II — Position includes tasks that

involve no exposure to blood, body fluids, tissues, but employment may require unplanned Category I

tasks.
