Full Job Description
As an IOP Case Manager for Aurora Behavioral Health Care, you'll provide direct and indirect patient care and case management services to active-duty military patients attending an intensive outpatient program to address Posttraumatic Stress Disorder, Mood Disorder, and Chemical Dependency Disorder.
*Pay Range: $30/hr. to $46/hr.
Responsibilities:
- Communicates with referral sources and other appropriate parties regarding patients’ progress
- Assists in the development and implementation of treatment plans
- Coordinates and implements patients’ discharge
- Attends and participates in department and other program or clinical and administrative meetings
- Supports maintenance and development of clinical program or services
- Conducts family therapy sessions when indicated
- Facilitates patient groups as scheduled
- Maintains effective communication with all necessary departments
Skills:
- Professional experience working with military families or personal military lifestyle experience
- Knowledge of the difficulties of dealing with the requirements of military life.
- Knowledge of military and community resources appropriate for patients
- Strong oral and written communication skills
- Ability to work effectively both independently and as part of an interdisciplinary treatment team
Requirements
- MUST posses a MASTERS degree of SOCIAL WORK (MSW)
- BLS for Healthcare Providers
