Full-time Licensed Professional Clinical Councelor
Gotham Enterprises Ltd are seeking to hire a compassionate counselor to join our team of dedicated healthcare professionals. The LCSW applies psychological/counseling principles in the following areas: assessment, clinical interventions, research, training, case management, consultation, program development and supervises non-licensed staff (when applicable). Responsibilities will include diagnosis, evaluation, treatment, discharge planning and supervise mental health services provided to participants. Provides a variety of interventions such as individual, group, couple and family therapy in the PACE setting. Involves integrating behavioral health services with the PACE interdisciplinary team and community agencies to ensure appropriate participant care as outlined in the care plan.
Salary: $110,000-$130,000 per year, plus full benefits
Responsibilities:
- Evaluate mental health diagnosis, create and implement a treatment plan, and complete ongoing documentation including further diagnosis, treatment plan reviews, and case notes according to company policy
- Periodically serve on-call during scheduled workdays for patient emergencies, provide consultation and crisis intervention services on an as-needed basis.
- Conduct initial mental health (MH) evaluations including criminal, psychological, and substance abuse case history to assess patient current needs and make treatment recommendations in the initial evaluation.
- Conduct clinically sound suicide risk evaluations, including clinical review, applying suicide risk assessment protocols, pertinent data/chart reviews, proper documentation, and consultations as needed.
- Meet with patients for crisis intervention, group psychotherapy, in-person interviews, etc. The clinician may be required to provide clinical, in-person interviews at cell fronts in inmate housing units.
- Identify the relevant signs and symptoms of psychiatric disorders in order to comprehensively assess, diagnose and manage the presenting symptoms, consults with medical personnel regarding the findings of medical examinations and evidence of organic disturbances related to behavior disorders, and with nursing, social work, education, custody and other staff regarding precipitating events, complications, and potential for adjustment.
- Write clinical reports regarding diagnoses and prognoses and develop treatment plans that reflect the standard of practice.
- Consult on complex cases with supervisor, treatment team members, CDCR/CCHCS institutional health care personnel, and other divisions within the department.
- Document all care provided to the patient, including in-person contacts and Interdisciplinary Treatment Team (IDTT) meetings.
Requirements
Qualifications:
- Professional Licensure as a LPCC by the California Behavioral Science Board.
- 1 year of experience as a LPCC within the last 3 years
- Valid CPR/BLS Card from the American Heart Association
- Proof of negative TB test within the past year
- Up-to-date immunization records
- Ability to pass a background check
