Position Overview: Your role involves conducting assessments, designing treatment strategies, and leading therapy sessions. You'll work closely with our care team to deliver seamless, integrated support.

Salary: $110,000 - $130,000 per year + benefits

Key Responsibilities:

• Perform detailed psychosocial assessments to understand client needs.
• Provide therapeutic support, using evidence-based methods for individuals and groups.
• Collaborate with healthcare professionals to develop integrated care plans.
• Advocate for client access to resources and services.
• Document client progress and treatment outcomes in electronic health records.
• Engage in ongoing training and team discussions to enhance your expertise.

Requirements

• Master’s degree in Social Work from an accredited institution.
• Current licensure as LCSW in the state.

·         Must have experience working with older adults.

·         Strong clinical and diagnostic skills.

·         Excellent communication and interpersonal skills.

·         Must be able to work independently and as part of a team.
