Job Details:
- Type: Full-time
- Salary Range: $110,000k–$130,000k annually
Key Responsibilities:
- Perform biopsychosocial assessments tailored to patients' age and cultural backgrounds in primary care settings.
- Deliver counseling services to individuals, couples, and families, including crisis intervention and treatment planning.
- Act as a liaison to connect patients with local resources and community programs.
- Support quality improvement initiatives by contributing to data collection processes.
Requirements
Requirements:
- Master’s degree in Social Work, Counseling, or a related field.
- Active California LCSW license.
- At least 2 years of clinical service experience.
- Proficiency in MS Office and electronic health records.
