Full Job Description
Under the supervision of the Director of BH, the IBH Care Coordinator is an essential member of the behavioral health-primary care care team. They are responsible for promoting improved access and care management for clinic patients who have medical and behavioral health care needs. This position also promotes timely and coordinated internal communications among clinic primary care providers, behavioral health specialists, consulting/treating psychiatrists and patients. In order to maximize patient access and provider productivity, this position also collaborates with clinic staff, manages provider schedules and client appointments, follows up with no shows, facilitates warm hand offs between medical and behavioral health care staff, and provides referrals for community resources to support whole person health. This position utilizes the electronic health care record and population management tools, to collect data, communicate with providers and produce reports to support collaborative care, improved health outcomes and program compliance.
The BH Care Coordinator has a lead role in the design, implementation and monitoring of clinical care programs to ensure the highest utilization for optimal health outcomes in affected patient groups. This may include but is not limited to the planning of clinical protocols related to BH, Chronic Care, and other health conditions, internal promotion through health education and leading of discussion groups among BH and Primary Care providers and Clinical Staff.
Compensation: $24.19 - $28.00 per hour, depending on experience.
Responsibilities:
- Accepts warm hand-offs of patients between agencies, medical and behavioral health providers to facilitate integration of care.
- Assists in establishing and maintaining smooth functioning and relationships between primary care providers, behavioral health specialists, and consulting psychiatrists.
- Tracks, monitors and reports client attendance and follows up on no-shows to support and improve continuity of treatment.
- Tracks, monitors, and reports depression and/or anxiety screenings and SUD screenings within primary care and behavioral health and follows up as needed.
- Review and adhere to eligibility requirements for services, and assist patients in navigating their care with both in-house and outside providers by facilitating scheduling of visits, identifying barriers to receiving care, and assisting with providing referrals.
- Participates in, under the supervision of the Director of BH, linkage for patients who are experiencing a crisis or require a higher level of care to facilitate the referral and confirm contact with the service provider.
- Provides health education and patient information as appropriate to promote the understanding and adherence to BH screening and treatment.
- Provide sensitive and accurate information to patients, including those being screened and treated for BH conditions.
- Participates in, coordinates, and/or attends and conducts outreach at community events such as health fairs and community forums, as a representative of TVHC, Inc.
- Responsible for data collection and generation of reports as directed by the BH Director.
- Attends regular meetings and provides oral and/or written reports on the status of clinical initiatives.
- Generates and produces a variety of correspondences, forms or other written items.
- Adhere to privacy and information security requirements (HIPAA) and other regulations. Also performs functions, duties and/or services in compliance with regulatory agencies, contractual obligations and funding source requirements.
- Maintain high level of respect and confidentiality in handling patient information at all times.
- Promote and support the patient centered medical home through collaboration with other departments.

Requirements
- Bilingual in English and Spanish required.
- Highschool Diploma, or G.E.D. required.
- AA degree required in health, human services, or social services field or equivalent experience of two years in a health care organization will be considered.
- Experience within an FQHC in working with patients and clinical staff, including case management, peer counseling, obtaining personal history, client health education and referral management preferred.
- Demonstrated ability to work effectively independently and as part of a team, in collaborative settings, required.
- Proficient in Microsoft Word and Strong preference for experience in EPIC.

- Ability to travel to designated location as assigned by supervisor.
- Demonstrated knowledge of and be comfortable presenting information related to behavioral health and chronic health conditions.
