Join the Masonic Homes Team!  We are looking for an Intake Program Manager

Location: Hybrid Office - 3 days WFH, 2 days On-site

Schedule: Flexible! Monday through Friday, 9 am - 5 pm.

Pay Range: $90,000 - $100,000/year, plus full benefits, 17 days’ vacation, and a 401 (k) bonus and matching!

JOB SUMMARY

The Intake Program Manager oversees the Masonic Assistance intake operations, leading a team of three staff. Your department's responsibilities include managing the intake process for new applicants and engaging clients through phone calls and digital inquiries. This hands-on role involves direct participation in intake calls and inquiries.

As the Masonic Assistance Intake Program Manager, you'll play a crucial role in our mission to provide comprehensive care services and case management support to adults, families, and seniors, directly impacting the lives of individuals across our community. You’ll be the go-to expert on Masonic Homes’ many programs and services, assisting clients with accessing support and guiding the team to ensure clients' needs are met with compassion and efficiency.

The Intake Program Manager reports to the Director of Masonic Assistance and Outreach.

 

ESSENTIAL FUNCTIONS

Manage Masonic Assistance Intake Program (50%)

  • Supervise Intake and Resource Coordinator and Masonic Assistance Social Worker, mentoring them through complex cases.
  • Oversee intake process for Masonic Outreach Services and Masonic Homes of California.
  • Ensure prompt and appropriate responses to Information and Referral calls.
  • Assess caller needs and provide immediate care management or referrals.
  • Collaborate with lodges and other affiliated organizations.
  • Use judgment to triage to appropriate relief programs and develop follow-up plans.
  • Obtain required documentation and conduct assessments of the physical, psychological, functional, financial, emotional, and environmental status of clients.
  • Track and report data for service evaluation.
  • Manage Masonic Homes and Masonic Outreach Services application workflow, reviewing applications and preparing documentation for approval.
  • Facilitate interdepartmental communication for timely service coordination.
  • Track and report on data to assess the quality and efficiency of services provided.

Support Masonic Assistance Intake Program (50%)

  • Conduct telephone assessments to evaluate caller status and formulate service plans.
  • Formulate preliminary budgets for pre-qualification, adhering to guidelines.
  • Provide information, referrals, and care management to stabilize at-risk situations.
  • Offer ongoing care management to guide applicants and connect them with resources.
  • Correspond with applicants, families, and lodges regarding application status.
  • Inform callers of available public benefits and assist in obtaining them.
  • Interact with membership, staff, and partners to ensure consistent messaging.
  • Responsible for Masonic Assistance Quality Assurance.

Requirements

  • Strong leadership and management abilities can guide a team to ensure efficient and effective service delivery.
  • A compassionate approach and creative problem-solving abilities, especially when working with aging and at-risk individuals.
  • Knowledgeable about county and community resources for seniors and at-risk populations, with a solid understanding of aging processes, health systems, and mandated reporting laws.
  • Strong communication skills, particularly over the phone, ensuring clear and supportive interactions with clients and families.
  • A dedication to exceptional customer service, ensuring every interaction is treated as a VIP experience.
  • Tech-savviness, with the ability to navigate digital tools for case management, application processes, and data tracking.

Here is what you will need:

  • Master’s degree in social work, psychology, gerontology, or related field.
  • Prior team management experience in a social services non-profit setting or government-funded program preferred.
  • At least 5 years’ experience in social services, case management, community-based programs, mental health, gerontology, children and family services, veteran’s services, or related fields.
  • A valid California driver’s license.
