Target Hiring Range: $ 74,579.76- $ 85,766.73 Annually

POSITION SUMMARY

Facilitates optimal quality and timely service delivery for CHG members along the entire continuum of care. Assists in promoting cost-effective utilization of services through anticipatory coordination of population health management; assistance with population health management programs, projects, and health education services, including literature reviews, program/project development and coordination, intervention implementation, health education and promotion, and evaluation. Collaborate with clinical and non-clinical providers, and community-based organizations to meet members’ needs, including health education and population health management services, and social determinants of health. Participates in organizational efforts to manage individual and population-specific care. Executes assigned projects in health education and promotion, population health management, cultural and linguistic services, and others.

COMPLIANCE WITH REGULATIONS

Works closely with all departments necessary to ensure processes, programs and services are accomplished in a timely and efficient manner following CHG policies and procedures and compliance with applicable state and federal regulations.

RESPONSIBILITIES

  • Assists Health Education Manager with planning, execution, and evaluation of preventative and health education services and projects, following nationally accepted guidelines, to improve health outcomes of CHG’s member population;
  • Conducts health education and promotion activities;
  • Identifies and utilizes available resources to improve services for members with high risk and/or complex cases.
  • Participates in coordination of population health management programs. Assists Health Education Manager, with condition-specific and other programs and projects.
  • Evaluates program and service utilization. Identifies service gaps among preventative, health education, cultural and linguistic, and population health management services.
  • Conducts health education and promotion activities and training on various health-related and regulatory topics, including the development of health education materials, videos, and other health promotion and training materials.
  • Maintains case documentation according to established guidelines and standards.
  • Contributes to the team effort, maintains customer confidentiality. Attends department meetings; provides feedback for existing processes; maintains confidentiality; representing the department in interdepartmental and external meetings and forums regarding each area of expertise; educating and serving as a health education and disease management resource to staff.
  • Maintains product and company reputation. Contributes to the team effort by conveying a professional image and accomplishing related tasks; participates on committees and in meetings;
  • Performing other duties as assigned or requested.

Requirements

Education

Possess either a 1) Master of Public Health (MPH) degree with a specialization in health education or health promotion, from a program of study accredited by the Council on Education for Public Health, sanctioned by the American Public Health Association; or 2) MCHES (Master Certified Health Education Specialist) awarded by the National Commission for Health Education Credentialing, Inc.

Experience/Skills

  • Bilingual in English and Spanish required.
  • Experience in a managed care environment, hospital, and/or outpatient clinic.
  • Knowledge of community-based programs on health education.
  • Current driver's license and proof of auto insurance.
  • Ability to effectively use Microsoft Office (minimally, Word, Excel, and PowerPoint), telephone, fax machine, photocopier.
  • Ability to communicate effectively verbally and in writing; exceptional telephone and customer service skills.
  • Ability to establish effective working relationships with physicians and medical professionals.
  • Ability to organize work effectively determines priorities, and work well independently.

Physical Requirements

  • Prolonged periods of sitting at a desk; intermittent standing, walking, bending, stooping, lifting 10 lb. or less.
  • May be necessary to work and attend meetings outside of facility or normal business hours.
