Seasoned Recruitment is currently seeking a dedicated and experienced Geriatric Primary Care Nurse Practitioner to join our team. As a Geriatric Primary Care Nurse Practitioner, you will provide comprehensive primary care services to elderly individuals, focusing on their unique healthcare needs.

Responsibilities:

  • Conduct comprehensive physical and psychosocial assessments of elderly individuals
  • Diagnose and treat acute and chronic medical conditions
  • Prescribe medications and order diagnostic tests
  • Develop and implement individualized care plans
  • Provide health education and counseling to patients and their families
  • Monitor and evaluate patient outcomes, adjusting care plans as necessary
  • Collaborate with other healthcare providers to ensure coordinated and holistic care
  • Adhere to regulatory and documentation requirements

Requirements

Requirements:

  • Master's degree in Nursing with specialization as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
  • Current unrestricted Nurse Practitioner license in the state
  • Minimum of 2 years of experience as a Nurse Practitioner in primary care
  • Experience working with elderly individuals is preferred
  • Ability to work effectively in a fast-paced environment
  • Excellent communication and interpersonal skills
  • Compassionate and patient-centered approach to care
  • Knowledge of geriatric healthcare and best practices
