Join Our Team as a Community Outreach Coordinator!

Are you an experienced Community Outreach Coordinator with a knack for sales? We are excited to welcome you to our dynamic and growing team! In this role, you'll spend most of your day off-site, forging strong relationships with community-based organizations (CBOs), referring physicians, medical offices, and other key community stakeholders in your assigned area.

What You'll Get To Do

  • Territory Management: Effectively manage your assigned territory, ensuring optimal coverage and engagement.
  • Education and Training: Educate CBOs and referring provider offices on new food offerings, enhancements, enrollments, and outcomes.
  • Issue Resolution: Identify and resolve service issues related to CBOs, referring physicians, their staff, and patients.
  • Network Growth: Grow the CBO and provider network through new provider recruitment and business development opportunities.
  • Presentation Preparation: Gather and prepare presentation materials for training sessions.
  • In-Service Organization: Organize and conduct office managers' in-service, including training, marketing, and education materials.
  • Activity Logging: Maintain detailed visit and activity logs in HubSpot, tracking issues raised during visits.
  • Escalation: Escalate unresolved issues to the manager/director for tracking and resolution.

What You'll Bring To The Team

  • 2-4 years of health plan or management service organization experience in CBO or provider relations. 
  • Bilingual English/Spanish.
  • Excellent oral and written communication skills.
  • PC literate, including Google Sheets, Google Docs, Canva, etc.
  • Strong customer service skills and experience.
  • Strong organizational skills.
  • Excellent interpersonal skills.
  • Ability to work in a team environment.
  • Ability to work independently.
  • This position requires local travel (within 50-mile radius) at least 80% of the time.

  • Compensation for this role is $52k + commission.
  • Unlimited PTO and 9 public holidays.
