Apollo Home Health is recruiting for a Clinical Care Coordinator to join our Home Healthcare Team. In this role, you will be responsible for providing support to field clinicians and assisting the Director of Nursing.  You will be joining a team of compassionate and dedicated healthcare professionals who are committed to providing the highest level of care to our members.

The Clinical Care Coordinator plans, supports field clinicians and directs home health services and is experienced in  home health nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Primary functions are to assist the Director of Nursing with office tasks, to assist with office tasks that require a Licensed Vocational Nurse (LVN/LPT) license. Ensure that patients are cared for according to the agencies policies and procedures and Best Practices.

Work as an assistant to the Director of Nursing in the following areas to include but not be limited to:

1. Receive incoming calls regarding clinical questions from patient and staff. Escalate to an RN case manager or DON if additional information is needed and then return the call the same day.

2. Oversee the OASIS transfer documentation, follow-up phone calls Hospital Hold status and Resumption of Care/DC of patients that are admitted to the hospital.

3. Review and sign Start of Care Orders, Recertification Orders, Resumption of Care orders, Discharge Orders and external orders.

4. Review and sign OASIS-Discharge and OASIS non-bill Discharges.

5. Manage the High Intensity patients (intake, scheduling, meetings with Optum, etc)

Requirements

Education: Graduate from an accredited diploma, associate or baccalaureate nursing program.

Licensure: Active, state license as a registered nurse. Current driver’s license.

Experience: Home health experience with knowledge of intake, scheduling, authorizations, OASIS, and orders.

Skills: Proof of current CPR certification. Expert in back-office tasks. Ability to communicate with others regarding home health issues. QAPI / Chart Review Experience.

Transportation: Reliable transportation and valid auto liability insurance.

    • Apollo currently pays CCCs $35.00 -$39.00

Salary Range Disclaimer:

    • The rates listed represent the low and high end of the range for this position. Visit and reimbursement rates vary based on visit type and employment status (1099 or W2). Rates may be adjusted in the future, and employees will be notified of these changes.
