Case Manager LCSW

We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team as a Case Manager in Santa Monica, CA. The Case Manager will provide comprehensive case management services to a diverse clientele, with a focus on mental health and social service needs.

Responsibilities:

• Conduct thorough psychosocial assessments to identify client needs and develop individualized care plans.
• Connect clients with appropriate community resources, including housing, healthcare, mental health services, and employment assistance.
• Advocate for clients' rights and needs, navigating complex systems and barriers to care.
• Provide crisis intervention and support as needed.
• Maintain accurate and timely documentation of all client interactions and progress.
• Collaborate effectively with a multidisciplinary team to ensure holistic client care.

Qualifications:

• Master's degree in Social Work (MSW) from an accredited program.
• Current LCSW licensure in the state of California.
• Experience in case management, preferably in a community mental health setting.
• Strong knowledge of community resources and social service systems.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work independently and as part of a team.
• Bilingual (Spanish/English) preferred.
