Position Summary
The Case Manager | Enhanced Case Management (ECM) will provide comprehensive case management services to individuals and families, focusing on enhancing their access to necessary resources and support. As a Case Manager, you will work closely with clients to conduct assessments, develop care plans, and provide ongoing support and advocacy. You will collaborate with community partners and agencies to coordinate services and referrals, ensuring the holistic well-being of clients.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Conduct comprehensive assessments to identify clients' needs and resources
- Collaborate with clients to develop and implement individualized care plans
- Provide ongoing case management support, including regular check-ins, goal setting, and progress monitoring
- Assist clients in accessing and navigating community resources and services
- Advocate for clients' rights and needs within the community
- Coordinate with community partners and agencies to ensure coordinated and holistic care
- Maintain accurate and up-to-date documentation of client interactions and service provision
- Participate in team meetings, trainings, and professional development activities
Requirements
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Bachelor's degree in social work, psychology, or a related field
- Minimum of two years of experience in case management or a related field
- Experience with enhanced case management models is preferred
- Knowledge of community resources and services in the area
- Strong communication and interpersonal skills
- Ability to build rapport and establish trusting relationships with clients
- Excellent organizational and time-management skills
- Proficiency in using case management software and computer applications
- Cultural competence and sensitivity to the needs of diverse populations

Hourly Salary Range: $21.15-$35.67
