We are looking for a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team! As an LPN, you will play a crucial role in providing essential, hands-on care across a range of healthcare settings, including hospitals, clinics, and long-term care facilities. Working under the supervision of registered nurses and physicians, you'll help to improve patients' lives by addressing their physical, emotional, and mental health needs with warmth and expertise.

In this role, you'll carry out a wide variety of clinical tasks, such as taking vital signs, administering medications, assisting with patient assessments, and participating in the development of individualized care plans. Key Responsibilities

  • Direct Patient Care: Monitor vital signs, observe patient conditions, and communicate findings with the healthcare team.
  • Medication Administration: Safely administer medications and treatments as prescribed by registered nurses and physicians.
  • Patient Care Planning: Assist in the creation and continuous updating of patient care plans tailored to each patient's needs.
  • Documentation: Maintain accurate patient records and documentation to ensure effective, seamless care.
  • Emotional and Educational Support: Provide patients and their families with education on health conditions and emotional support as needed.
  • Team Collaboration: Work with healthcare professionals to enhance patient outcomes and contribute to effective discharge planning.
  • Environment Maintenance: Ensure a clean, safe environment, strictly adhering to infection control and safety standards.

Requirements

  • Current LPN licensure in the state of practice.
  • Completion of an accredited LPN program and relevant nursing education.
  • Basic Life Support (BLS) certification or equivalent.
  • Strong clinical assessment skills and attention to detail.
  • Excellent communication and interpersonal skills for effective patient interaction.
  • Ability to work effectively in a team and independently as needed.
  • Flexibility to work varied shifts, including nights, weekends, and holidays as required.
