Position: Full-Time
Schedule: Monday to Friday, 9 AM – 5 PM
Primary Duties:
- Assist individuals facing mental health and social barriers to improve their well-being.
- Collaborate with healthcare professionals to ensure seamless care coordination.
- Establish partnerships with local agencies to connect patients with community resources.
Salary: $90,000 - $110,000 per year, plus comprehensive benefits.
Requirements
Qualifications:
- Master’s degree in Social Work or a related field.
- Active LCSW license.
- Proven experience working with aging populations.
- Strong skills in clinical evaluations and care planning.
- Effective communication and team collaboration abilities.
