We are seeking a compassionate and experienced Nurse Practitioner to lead the medical aspects of our team-based care for individuals with serious mental illness. Job is located in Tacoma, WA
Responsibilities:
Perform a whole-person clinical assessment, including:
- Gathering information from outside sources, especially other care providers
- Assessing both behavioral and physical health conditions through a thorough history and physical examination
- Capturing undiagnosed conditions, including ordering additional studies as needed
- Organizing information to support successful disability applications if appropriate
- Conducting visits in the field or in an individual's home when necessary
