Job Description

REPORTING RELATIONSHIPS: Reports to Managed Long term Care Plan and shares a collaborative relationship with the Nurses and other departments involved in member’s care.

PRIMARY RESPONSIBILITIES:

· Assists in scheduling of ongoing member visits such as UAS Assessments for the Field Assessors which includes tracking and rescheduling of canceled appointments.

· Must reach daily scheduling quotas.

· Provide a daily report of all scheduled visits.

· Assists with uploading all UAS documents into members files accordingly.

· Generates member correspondence and handles mailings as needed.

· Coordinates communication between Field Assessors and other members of the multidisciplinary team
