Substance Use Navigator - Turlock, CA

We are looking for a full time temporary (12 months) Patient Care Navigator/Substance Use Navigator (SUN) to work in an emergency department in Turlock, CA and Modesto, CA. This is a grant funded position that has 12 months of funding. We are looking for someone who can work Monday through Friday from 10 a.m. to 6:30 p.m. The week will be spit between each site.
