The Gathering Place (Brattleboro Area Adult Day Services), is an Adult Day program serving Windham County Vermont. We serve adults with a wide range of physical and cognitive abilities. Our participants enjoy music, arts, games, exercise and companionship - just to name a few!

We are seeking a community-minded Registered Nurse to provide oversight, integration and coordination of comprehensive health services and rehabilitative programs that meet the needs of The Gathering Place program participants.

Primary duties include:

• Completing health assessments for all participants.
• Outreach to participant care team members (primary care providers, case managers, caregivers, etc) to obtain, review and document clinical information to ensure a compliant and complete participant record.
• Provides caregiver support and assists participants/caregivers in accessing and utilizing community resources.
