The Director or Senior Director will be a foundational role as we continue to foster healthy communities and promote health equity through the delivery of high-value health and wellness services. This leader will be responsible for providing oversight and direction for designing, implementing, and supporting a robust population health program across the organization. By working collaboratively in a matrixed manner across clinical, operational, and business teams to improve population health, patient experience, care team experience, health equity, and reduce costs, this leader will strategically position TVHC for continued clinical transformation and value-based care.

This clinical position oversees Tiburcio Vasquez’s Health Centers (TVHC) Population Health Department, whose mission is to improve the health of our patients and our community by addressing disparities through health education, patient empowerment, and advocacy.
