Touchstone Health, a full-service home health agency, is seeking a qualified and compassionate Physical Therapist to care for our patients. Under the supervision of the Clinical Manger and in accordance with the interdisciplinary plan of care, our therapists evaluate, establish, and implement individual programs of care for our patients to increase functional ability and reestablish independence at home. Travel San Antonio and surrounding area.

Duties & Responsibilities:

• Performing home visits to evaluate patient's level of function
• Developing an individual plan of care based on the client's personal goals
• Implementing the treatment program through direct treatment of the patient
