The Waivered Services Case Manager assists clients on a waiver to access needed waiver and State plan services, as well as needed medical, social, educational and other services, regardless of the funding source. The Case Manager supports clients who have been deemed at risk of placement in a Nursing Facility, Neurobehavioral Hospital or ICF/DD, and sets up and coordinates the services and medical equipment necessary in order for them to live in the least restrictive setting possible. The Case Manager also provides ongoing monitoring of services, and serves as an advocate for the client when assistance is needed or services are not being performed as required.
