Case Manager II - Youth Programing

Position Summary

The Case Manager (CM) will identify, screen, and assist individuals with accessing culturally affirming social support services. This person will work in collaboration with community partners to conduct compassionate outreach to hard-to-reach BIPOC Youth and Young Adult residents ages 16 - 24 who may be experiencing multiple barriers to care (i.e., housing, food insecurity, transportation, etc.) In the role, the CM will focus on the whole person and administered intake, assessments, strength-based case management, advocacy and community-based referrals, and linkages.

Hourly Salary Range: $21.63-$26.44 plus $2 bilingual differential
