Care Coordinator - Children and Family Services

We are looking for a dedicated Care Coordinator to assist us with Children and Family care coordination at our Mommy & Me programs in Seaford & Lewes, DE and our Women's Residential program in Seaford, DE. The Care Coordinator - Children & Family Services will be responsible for consulting with residents and determining their needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible patient care. Ultimately, you will have a compassionate nature, be knowledgeable about health care practices and provide exceptional customer service to assist patients in handling their illness.
