The hospice receives patients from referring institutions throughout New York City and the entire tristate area. Calvary Hospice provides end-of-life care to patients with life-limiting illnesses whose care has transitioned from active curative to palliative and quality of life focused. The majority of care is provided in the patient’s home and as needed, short term inpatient care is provided at several facilities throughout the greater New York area.

Requirements

Assists in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families.

Salary: $54.65 - $68.71 per hour
