ACP Patient Advocate (RN or LCSW)

The ACP Patient Advocate is a vital member of the Koda Health care team, guiding patients and their loved ones through meaningful, longitudinal conversations as they navigate serious illness. In this role, you will work closely with complex patients in a virtual/telephonic setting to:

1. Define their values and ensure those values are shared with their loved ones.
2. Provide ongoing support and check-ins to ensure patients receive the care they value as their health needs evolve.

Additionally, you will screen for palliative care and hospice needs and effectively communicate these to the care team.
