Job Summary

The LVN will provide direct nursing services to patients struggling with substance use disorders. Responsible for managing detoxification, ongoing assessment, evaluations, triage, harm reduction teaching, stabilization, and monitoring of clients/patients in the ambulatory community-based treatment model. Develops goal-oriented treatment plans with clients/patients; collaborates with primary care providers, integrated treatment team, and community providers; as well as support and services to patients from other settings referred to the program for stabilization and assessment.

Plum Creek Recovery Ranch is a behavioral health facility serving the Central Texas region on a beautiful 200 acre ranch in Lockhart, Texas. The LVN will work directly under the supervision of the DON and in conjunction with the mental health care team. Plum Creek offers both detox and residential services.

The LVN is responsible for delivering quality nursing care to patients who are in treatment for substance use disorders; to support the program philosophy of care; to ensure the safety and well-being of each patient. The LVN will provide professional nursing care to patients according to facility policy and procedure; implement nursing care procedures, evaluate patient progress and modify care plans as necessary; will provide teaching to patients.
